- Within 10 days of arrest for DUI, request a hearing with the Office of Administrative Hearings in Hunt Valley, MD to request an administrative hearing by certified mail
- Enclose a check for $125 made payable to the Maryland State Treasurer along with the hearing request form which is a carbon copy of the temporary license that should have been provided to you after your arrest and processing by the police officer
- Failure to request a hearing with a postmarked envelope within 30 days will likely result in the suspension of your license on the 46th day from your arrest
